Sorors and Amicae

Please find links to two  Microsoft word Z-HOPE Reporting Forms. 

Form Z-HOPE 1 is created in Microsoft word where you will only need to click on the gray squares to input your information.  If your computer prompts you to click ok for file conversion please do so. 

Form Z-HOPE 2 is created in Microsoft word dot matrix format and  includes some information in drop down box format.   You will need at least the XP version of Microsoft to open and use this version. 

Use whichever form you feel most comfortable with completing.   There is also an option on the form to receive a confirmation e-mail from me. 

I hope this makes your Z-HOPE reporting a bit faster and easier.  Please share with your Z-HOPE coordinator for your graduate chapter, undergraduate chapter and auxiliary group.
